Description:
Hospice care is designed to provide patients with comfort, symptom management, and support when they are no longer seeking curative treatment. Medical and support services are provided to patients and families by a coordinated, interdisciplinary team of highly skilled and dedicated people working from each patient’s individualized plan of care.
The hospice team includes physicians, nurses, therapists, social workers, aids, chaplains, volunteers and other healthcare professionals. We collaborate with patients and their families to provide relief from suffering through pain and symptom management, and to also provide emotional and spiritual support.
While most patients receive hospice care in their home, hospice care can be provided at assisted living facilities, group homes, nursing homes, and at hospitals
