Transit Plus provides transportation services to people who are unable to use Milwaukee County buses and are found eligible for paratransit services. To become eligible for Transit Plus applicants must provide medical documentation and schedule an in-person assessment at their facilities located at 1942 N. 17th Street. Complete the application form, and have your licensed medical provider (physician, registered nurse, occupational therapist or physical therapist) complete the Medical Documentation Form. Bring them with a current form of photo identification to your assessment. If you need an individual to accompany you to provide assistance, such as a Personal Care Assistant (PCA), the medical provider should indicate this need on the medical documentation form. The assessment process takes place at the Transit Plus office in less than 45 minutes. The ride to and from the Transit office is the applicant's responsibility. In some cases, the Milwaukee County Office for Persons with Disabilities at 289-3933 can assist applicants with a ride to the assessment.
Eligible participants are provided with a "Rider's Guide" handbook describing the program in detail including telephone numbers to schedule rides. The Rider's Guide is available in Braille, large print, and audio tape.
HOURS OF SERVICE: RIDES PROVIDED--4:30 a.m. to 1 a.m., 365 days a year. RESERVATIONS ACCEPTED--8 a.m. to 5 p.m., 365 days a year. Authorized COMPANION RIDERS AND PCA'S MUST BE SCHEDULED AT THE TIME THE RIDE RESERVATION IS MADE.
ASSISTANCE: Van drivers can meet riders at the first set of exterior doors and assist riders to the vehicle; this includes support when walking, assistance in getting wheelchair riders up and down ONE step or ramps approved by local building codes and pushing wheelchair riders to and from the building entrance. Riders who have unique accessibility situation or require a higher level of assistance than the "over one step, through one exterior door" policy should contact the Office for Persons with Disabilities at 414-278-3933.
