Description:
Health Care for the Homeless Program is a system of clinics and services for homeless women, men and children in Milwaukee County. Services are located at meal sites, shelters, homeless services agencies and other community locations. HCFH provides Community Support Program (CSP) services inclusive of health, mental health, addictions, outreach, case management, advocacy and transitional housing services. The program also offers Homeless Outreach Workers who help homeless singles apply for SSI quicker than traditional services.
