Transportation of military veterans to Veteran Administration medical facilities in Milwaukee, Appleton, and Green Bay. The transport for the city of Green Bay is usually twice daily on an "as needed basis". The transport to Milwaukee is provided Monday through Friday and departs no later than 6:45am. Appleton is on an "as needed" basis and you will need to call as early as possible prior to your appointment to schedule transportation. All vans leave from the VA Regional Outpatient Center at 2851 University Avenue, Green Bay 54311. When calling for transportation, please leave your name, address, phone number, date and time of the appointment and the last four digits of your social security number. There is no cost for this service. Must be a military veteran.
