Description:

Camplbell House is a class CA, 6-bed CBRF, located in rural Sauk City in Sauk County. It provides quality residential services to residents with traumatic brain injury, cognitive impairments and mental illnesses. the staff also work with correctional clients who may be at risk for inappropriate or illegal sexual contact with others. The facility provides support, structured activities of work and leisure, medication monitoring, supervision or assistane with personal care, hygiene, and adult daily living skills. Transportation is provided and is included in the monthly rate.



Hours: 24 hours-Staff sleep nights



Eligibility: 18 years old or older: Must be ambulatory or able to maneuver w/o assistance; must be free of communicable disease; capable of doing ADLs
