Home Care Path LLC serves seniors who need a little help from a friend to live independently, but require companionship, non-prescription medication reminders, light housekeeping, and help with errands to remain in their homes.
Home Care Path LLC helps seniors who experience difficulty walking dressing, talking, eating, cooking, climbing steps, remembering medications, setting up bath, oral care, hair care, reading mail, washing dishes, discarding expired food, taking garbage out, consistent budget and finances, always alone/isolated, weight loss, changes in cognitive functions.
We help by: coordinating staff to assist with daily chores, preventing the senior from getting so run down they end up in the emergency room or permanently placed in an assisted living facility. Services are available up to 24 hours a day.
Indirectly contacts on individual baisis with local CMO's - Care Wisconsin, also contacts on individual basis with private insurance. Private pay rate for 2012 is $20.00/hour.
