Description:
The program provides support to homeless individuals 18 years and older who also have a mental health diagnosis. These individuals can be living on the streets, shelters throughout Brown County or about to lose their housing.
Services include: finding affordable housing, finding appropriate homeless shelter, mental health referrals for counseling and psychiatric appts., help applying for Social Security disability benefits, transport to doctor appointments, refer to AODA counseling services, connect with payee services, provide follow up case management for up to 60 days, provide bus passes.
