Part of the Dept of Human Services, the Children’s Long Term Support Waiver program serves children and young adults under the age of 22. In order to be eligible for programming, the child must have a significant developmental, physical or emotional delay which creates substantial limitations in areas such as self-care, language skills, learning, mobility, self-direction and/or emotional self-regulation. The program has a wide variety of services available and each service plan is developed in an individualized manner to meet the specific needs of the child and their family. The supports or services are designed to help children remain in their own home or community. Some of the services provided include respite, day services, adaptive aids, transportation, home and vehicle modifications, supportive home care and mentoring. Kewaunee County currently has a wait list for waiver services, but if your child may benefit from programming, please contact Melissa Annoye to begin the assessment process. In order to be served by the program, the child must meet a functional level of care, the child must apply for Medicaid and the child must be residing in an eligible setting.
