Description:
The Alzheimer's Family and Caregivers Support program was created in response to the stress and needs of families/and caregivers who are caring at home for someone with Alzheimer’s disease or a related dementia.
To be eligible, a person must have a diagnosis of Alzheimer's disease or a related dementia disorder and be financially eligible. There is no age requirement. In Rock County, up to $2000 per year is made available to each family in the program. Participants may receive a variety of services offered through various providers of service throughout the county. Available services include but not limited to: Day Care, Respite, Supportive Home Care, Home Delivered Meals, Life Line, Personal care, Transportation. To be eligible, you must be a Rock County resident and meet income eligibility.
Due to limited funding, a waiting list may exist for these services. Every effort will be made to provide funding as quickly as possible.
