Description:
Nurse practitioners are available to provide non-emergency medical care and case management to the homeless. Some services available include: primary health care, preventive health care, well-child physicals and immunizations, HIV and STD testing referrals, mental health referrals, help with some prescription medication, help with emergency dental care and eye care, and others. To be eligible you must: living in a shelter, living on the street, living in a room without kitchen facilities, temporarily living with a friend or relative, have an eviction notice. Free to eligible people. Hours for the nurse practitioner are posted at the agencies involved in the program.
