Care management integrates health, home and community services to meet the long-term care needs. They possess knowledge about costs, quality, availability of services and can assist or arrange in obtaining services a person is assessed for or as needed.
Referrals for My Choice Family Care Case Management for individuals 60 years or older must come through the Aging Resource Center of Milwaukee County by calling 289-6874 or for disabled individuals 18 - 59 years of age through the Disability Resource Center at 289-6660.
