Description:
Lifeline is a personal emergency response service which provides reassurance to subscribers and their loved ones 24 hours/day, seven days/week. By pushing a small help button that is worn on the person, help may be on the way immediately. The subscriber provides up to three people to respond for them in an emergency. This may be a friend, family member, neighbor, etc. The person should live in relatively close proximity to the subscriber's home to assure timely assistance. Lifeline service works through the subscribers home telephone line.
Eligibility: Lifeline is designed to serve the elderly as well as individuals with illness, injury or disability which makes them at risk.
Area Served: Portage County and Iola/Scandanavia area. Availability beyond these areas will be considered.
Intake procedure: Application form may be picked up at the hospital or requested by mail.
