Description:
The Alzheimer's Family & Caregiver Support Program (AFCSP) assists families who are caring for someone with irreversible dementia. The program can fund a wide range of goods & services that are necessary to maintain an income-eligible person w/ Alzheimer's disease in the community.
Eligibility depends on the medical & financial status of the person being cared for by the family.
Your family may qualify if:
You have a doctor's statement that you affected family member has been
diagnosed with Alzheimer's disease or any of the other irreversible dementias: and
Your affected family member's net income is below $40,000, after deductiong the cost related to the Alzheimer's care.
Subject to the availability of funds, AFCSP pays for goods & services necessary to allow an affected person to continue living in the community. Goods provided have included nutritional supplements, security systems, specialized clothing, home-delivered meals, hobby equipment & chair lifts.Typrical services have included in-home help, respite care, adult day care & transportation. The affected family member is expected to contribute to the cost of services based on ability to pay. Each person is eligible for AFCSP is allowed a maximum of $4,000/yr, if funding is available
