Description:

The Aging and Disability Resource Center of Waukesha County (ADRC) is a single access point for publicly funded long term care in Waukesha County. The ADRC provides information, assistance, counseling and supportive services for older adults age 60 and above; caregivers; adults with physical or developmental disabilities; and adults with mental health or substance abuse concerns.

Programs funded through Older Americans Act, State of Wisconsin, DOT, County Tax Levy, Elder Abuse Direct Services/prevention, and grants.

Family Caregiver Support Program

Information, counseling, respite, personal care/bath, medication management to assist caregivers. Extensive Caregiver Resource Library offering loan of books, videos, demonstration items. Programs available for grandparents parenting grandchildren.

Provides brochures, support group information, assistance, referral and case management to families involved with a member who has dementia/Alzheimer's disease.

For disability services provides home modification information and advice on home safety.

Volunteer opportunities available to set-up, serve, pack mals, register at senior dining centers; deliver meals to homebound or bring people to dining centers to eat; assist individual in sorting through bills, Medicare, legal issues, social security, etc.; visit older adults; help with office clerical and data entry; newsletter, assess/interview older adults for services; greet visitors to courthouse.

Information on pet probate available.

Reports of elder physical, material or emotional abuse, neglect or self-neglect which do not occur in a licensed facility or a licensed home care agency are investigated. Callers who report abuse may choose anonymity.

Benefit Counseling: assists in Medicare and medical bills, Social Security, Supplemental Security Income (SSI), Supplemental Medical Insurance, Housing - tenant/landlord problems, and eligibility rules in application for Title 19 benefits, Spousal impoverishment laws, Reverse Mortgage counseling, Guardianship, Advance directives (living will, power of attorney health care) and Power of Attorney information, and other legal/benefit problems. No charge; however, donations are requested.

Nutrition Services

Home delivered meals and Senior Dining Centers.

Nutritious meals (hot and cold) which are delivered to homebound older people with little support.Suggested donation $4.00 or whatever affordable.

Meals are delivered between 11am & 1PM, Monday thru Friday.

Eligibility: Homebound persons 60 yrs and above who do not leave home under normal circumstances.

Rideline

Lift-equipped vans provide specialized transportation for disabled and older persons. Trip based on priorities: 1)medical, 2) nutrition, 3) work-related, 4) social and respite.

Days/Hours: 6:00 am - 6:00 pm Monday thru Friday/no Saturday or Sunday transport.

Cost: Fare determined by origin, destination and ability to pay.

Eligibility: Non-driving Waukesha County residents age 65 years and older and individuals under 65 years who use cane, walker, crutches, wheelchair, or scooter, or are legally blind.

Reservations: at least 1 to 2 days advance notice is required.

Share-Fared Taxi Program: (service - up to entry door)

Subsidized fares for taxi service in the communities of: Oconomowoc, New Berlin, Waukesha, Elm Grove, Brookfield, Butler, Hartland, Nashotah, Delafield & Merton.

Eligibility: Waukesha County residents, non-limited drivers, age 65 or older OR Waukesha County residents, non-drivers, ages 18-64 who receive SSI or SSDI.

Cost: Rider pays the first $3.50 and any amount over $9.00.
