The Bureau of Assisted Living, Division of Quality Assurance determines the need for licensure of a community living arrangement based on the following type of facilities: Adult Family Home (3 or 4 residents), Community Based Residential Facility, and Residential Care Apartment Complex. The Division of Quality Assurance (DQA) is responsible for assuring the health, safety, and welfare of the citizens of Wisconsin. If any individual believes that a caregiver, agency, or DQA regulated facility has violated State or Federal laws pertaining to regulated entities, that individual has the right to file a complaint with DQA.
If you wish to file a complaint about a resident's care in one of a state licensed facility, you should make the complaint known to the facility's administrator. If not satisfied with the resolution, you may consider using the facilities required grievance procedure as a first option. You may also contact the Bureau directly for filing a complaint.
