Description:
The Family Caregiver Support Program provides information and assistance to family members, neighbors and friends who Family Caregiver Support Program care for adults 60 years old and older and persons with Alzheimer's disease or related disorder (regardless of age); grandparents and relative caregivers of children no older than 18 and relative caregivers of an adult child 18 to 59 with a disability.
The program consists of:
Assistance: Assessing particular care-giving needs, helping identify options and providing referrals to local community-based services
Information: Providing group presentations and public education to help caregivers learn of existing services and resources in their community
Respite Care Services: Funding to provide short-term and temporary breaks for the caregiver that can be provided in the home, adult day care or a facility
Supplemental Services: On a limited basis to complement the care provided by the caregiver, provide resources for home modifications, adaptive equipment or needed medical supplies
Training and Support: Assist caregivers in making decisions and solving problems related to their care-giving role and providing classes and support groups
