Chrishaven Group Home provides community based living environments in modern duplexes located within the area of beautiful La Crosse, Wisconsin. Residents learn and practice skills to foster independent living, enhance social skills, and become active participants in their community. Chrishaven is a non-profit corporation with a volunteer Board of Directors comprised of community members.



AFHs are 3-4 bed facilities. Respite Care is also provided.



Chrishaven Adult Family Homes serves individuals with diagnoses of cognitive disabilities with the primary conditions being Mental Retardation and the associated behavioral and emotional difficulties and individuals with autism spectrum disorders. Age served is adults. Intellectual functioning from mild to severe cognitive disabilities.



Individual diagnoses may include: Neurological Impairments; Learning Disabilities; Ambulatory Motor Deficits; Autism Spectrum Disorder; Traumatic Brain Injury; Epilepsy & Seizure Disorders; Language Delays; Down's syndrome; Fragile X Syndrome; and Pervasive Developmental Disorders.



Referrals are received from a wide variety of sources that initiate the admission procedure. Referral packets should include: Medical & Social History; Current Program & IEP Information; Psychometric testing (when available); Psychological and/or Psychiatric Reports; Recent Physical Exam; Medication History; Neurological Exam When Appropriate; and Description & Frequency of Current Behaviors and Emotional Problems.



Admission candidates are evaluated by the Director, Social Worker, House Coordinators, Client Coordinators and others, as deemed appropriate. Referral materials are used to establish programming, and often a personal visit is scheduled prior to placement to aid in the transition process.



Service & Goals:

Personal - Technical Assistance, Training, and Supports in Learning Functional Life Skills; Health & Safety Awareness Training; Training and Assistance in Managing Personal Affairs; Self-Advocacy Skills Training; Orientation and Community Transportation Skills Training; Utilizing Community Resources Training; Socialization Skills Training; and Leisure Skills Training.



Social - Structured Recreational Programs; Supportive Dialogue with Staff; Behavior Management Programs; Stress Management Programs; Coping Skills Training; Learning Environmental Adaptations; Counseling Services; Social Skills Training; Sensory Integration Programs; and Socialization Skills Training.



Education & Vocational - Individual Tutorial Services; Workshop Support Services; Technical Training and High School Equivalency Classes Available; Habilitation Services (Occupational, Speech, and Physical Therapy); Public School & Workshop Programs; and Supported Employment.



Medical Services - Medical services are coordinated through Gunderson Family Health Center under the direction of Chrishaven medical director. All Services provided are evaluated on a yearly or as needed basis and incorporate the evolving goals and objectives of each individual resident.



Program Goals - Development of Independent Living Skills within the Home, Work, or School Environment; Development of Motivation and Opportunities for Leisure and Recreational Activities; Improved Socialization Skills and Peer Involvement; Successful Vocational and Educational Experiences; Pride in Self, Home, and Community; Understand & Implement Self-Advocacy; and Ability to Understand & Utilize Community Resources.



Staffing Patterns - Residents are offered technical assistance and supervision through a rotating shift of paraprofessional staff. Staffing patterns generally consist of 3 staff members to 8 residents during the waking hours, with 1 awake overnight staff during sleeping hours. Chrishaven insures adequate staff to meet the needs of each resident, and draws from all available community resources in the implementation of program services.
