Description:

Evaluations are conducted at Ministry Door County Medical Center Rehab Service in the Cherry Point mall and at Good Samaritan Scandia Village in Sister Bay. In order to receive a driver's evaluation a patient must be referred by a medical doctor. Patients and families often request these referrals or are directed by their doctor to have an evaluation. Driving evaluations are not covered by Medicare or private insurance. Cost for the service is $110.00 prior or during service.



Hours are by appointment only. Typically they set aside Tuesdays for the evaluations, but can be done on other days. Appointments generally last an hour. Their typical timeframe on a Tuesday is 12:00 PM til 3:00 PM but again, that is not set in stone.
