Stewart Street CBRF is designed to support individuals who fall into one or more of the following populations: Advanced age, developmentally disabled, emotionally disturbed/ mental illness, physically disabled or traumatically brain injured individuals. This home is equipped to support individuals who cannot self-evacuate in less than 2 minutes.
The home is accessible and meets the needs of individuals requiring mobility and transferring devices.
Home is located in a quiet neighborhood of Richland Center, Wisconsin. It has ample room to comfortably accommodate equipment needed to meet individuals' special needs. Private and shared rooms are available. Common rooms are accessible to all individuals; bathing facilities accommodate complex transfer needs.
Supervision, activities of daily living supports, health monitoring/management, personal cares, management of medication and/or therapeutic protocols, financial management assistance, professional appointments coordination and escort, transportation, leisure activity supports, community integration, coordination with outside agencies and other services as identified to meet individual needs- All services are provided to the level indicated in the individual service plan.
This is a 24 hour awake- staff home, with double staff 16 hours per day and single staff during the night. Additional staff to accommodate community integration activities and any changing needs of consumers is responsive in nature. A residential coordinator is assigned to manage the day to day consumer and residential concerns. All staff receives initial and on-going training as mandated per federal and state governmental regulatory entities and as appropriate for the specific needs of the individual.
