Description:
Saint Jude Hospice proveds the patient and families the opportunity to participate in decision-making process and to focus the care on symptom management, comfort, dignity, and quality of life. The care is provided by an interdisciplinary team including nurses, aides, social workers, chaplains, volunteers, massage & music therapists. Persons eligible for for hospice include individuals who have a life-limiting illness, have the consent of their family physician, and have decided that comfort care is their wish. Hospice care is provded wherever the patient calls home, whether it's their own home, assisted living, or nursing home. Medicate, Medicaid, Badgercare, Family Care, and many private insurances have a hospice benefit.
24-hour on-call nursing. Bereavement follow-up for family for one year following the death.
