This 4-bed adult family home (AFH) is located in a quiet neighborhood. Layout of home offers residents the opportunity for family gathering or quiet space, a newly remodeled bathroom easily accommodates wheel chair and other equipment. Respite services are considered when a bed is available. Bedrooms are private.
The AFH is licensed to support Advanced Age, Developmental Disability, Emotionally Disturbed/Mental Illness, Physically Disabled and Traumatic Brain Injured. Individuals who are associated with the client groups supported in this AFH are considered compatible when health, safety and welfare support needs are similar. The home is accessible. Hearing and Visually impaired individuals may be supported. English is the spoken language.
The community offers several opportunities for day services, employment, entertainment, shopping, hair care salons and professional services, including medical, mental health. Both private and public funding is accepted. The goal of this AFH is to support individuals to maintain their ties in the community and encourage an active lifestyle.
Services offered include assistance with activities of daily living, meals, personal cares, medication management, health monitoring, recreation and leisure time supervision, reasonable amount of transportation, and information and referral services.
Service specialties include the support of individuals with behavioral and communication challenges, medical and transfer complexities. Direct support staff meets the training requirements as per DHS 88. Staffing patterns are dynamic dependent on individual and group resident need, activity and health, safety and welfare concerns. RN supervision and delegation to direct support workers will meet the requirements of the individual, contractual agreements and regulation. Staff and program needs are coordinated and supervised by a full time residential coordinator. Individuals who would like consideration for admissions will need to go through a formal referral and assessment process.
