Skilled Nursing Facility (172 Bed) with Dementia care units, Short Term Rehabilitation services, Hospice Care, Respite Care.
We also provide services for the chronically mentally ill. Psychiatric services are available on a weekly basis. Outpatient Physical and Occupational Therapy Services are available to the community.
A 16 bed facility, "Life Ventures," offers active treatment, rehabilitation hospice care and respite care to those developmentally disabled adults. Individuals range from young adults to the elderly.
Most have severe physical disabilities and are medically fragile. Others have behavior problems who need to improve skills for interacting with others. Provides Respite care, Rehabilitation and Hospice Services. We provide 24 hour nursing service, primary and psychiatric physician visits.
We Provide active treatment-teaching daily living skills, vocational and community integration. Life Ventures goal in each individual will be physically healthy, as emotionally stable, as mentally adept, as self-responsible, as vocationally capable and as independent as his/her capabilities allow.
8-bed CBRF unit (Woodland Care) for MI/DD Population.
Adult Day Care Services (Willow Ridge) for MI/DD and dementia. Population (day "respite"services).
