Provides an individualized assesment that includes: a detailed history , a physical assessment, cognitive screenings, in-home functional assessment, recommendations for care, patient/family education, follow-up support.
Other tests may be arranged based upon individual needs. Each situation will be evaluated to determine the most appropriate course of follow-up.
