Description:

KOINONIA is a 35 bed facility providing primary treatment to adults who are experiencing substance abuse. The name KOINONIA means 'Fellowship around a common goal' and reflects that recovery from addiction should occur in supportive, therapeutic environments with emphasis on quality of care. Accepts cash, checks, money orders and some insurance.

Area served: Koinonia is a contracted provider for the human service center as well as numerous other counties. Koinonia is also a preferred provider for a number of insurance plans. Self pay is also accepted.

Koinonia counselors are dually credentialed to address both substance abuse/mental health needs. Specialized Programs: Native American Culture; Family Program; Social Setting (non-medical) Detox Program; Relapse Prevention Program; and Koinonia Alumni Group, and Gender Specific programming.

Fees: KOINONIA provides primary addiction treatment at residential rates. These rates include: all facility program offerings psychological evaluations, food, lodging, recreation, and all routine programs costs. Contract rates are available and may be arranged through the facility.

Items to bring upon admission:

*Social Security Card or current ID so we can apply for a card while you are here

*Medical Insurance, Medical Assistance or Medicare card for any health care you may need while at KOINONIA

*If on medication, please bring a supply of these medications with you on day of admission. Before arriving check with us to see if those medications are allowed at KOINONIA. You must have current doctor's prescriptions. Also, because we are a residential facility, you will need to have a physician's prescription to routinely take over the counter medication for a specific condition. Also bring any pertinent medical records.

*Current financial information if from Forest, Vilas or Oneida County.

*Checking/savings account # and current balance

*Clothing: Bring no more than 4 to 5 days change of clothing. Include outerwear appropriate for the season. NO spandex or other inappropriately tight clothing, no alcohol/bar or offensive message clothing.

*Addresses/phone numbers of family members and close friends, your lawyer, probation/parole agent, referral source, and addresses of prior treatment centers, etc. We need this for release of information.

*Alarm clock or small radio alarm.

NOT ALLOWED: No electronics. No audio or video devices, camera or CDs, radios, CD players. No printed material that is not related to recovery. No products containing alcohol such as cologne, perfume, aftershave, aerosol sprays, etc., and no vehicle.

Services Provided: KOINONIA provides the following services as a part of our overall chemical dependency treatment program. These services include, but are not limited to the following:

*Health monitoring service

*Leisure/recreational activity service at YMCA

*Transitional living service

*Supervised medication when needed

*Counseling service

*Transportation service

*Money management service

*Prescribed personal care

*Information/referral service to obtain services required that are not directly provided by KOINONIA

-Vocational guidance counseling

-Education and/or employment training

-Legal aid

-Health Care

-Mental Health services

-Other required services

These services are provided based on individual client assessment and their individual needs which is reflected in the client's individualized treatment plan.
