Description:
Offers advanced-scheduled, door-to-door transportation to eligible older adults to medical and dental appointments, for grocery shopping, to the Senior Meal Program/Senior Centers.
Eligibility Criteria: You must be a Milwaukee County Resident age 60 or older, Able to walk but unable to drive, have difficulty in using public transit, must not be currently eligible for Transit Plus or Milwaukee County's para transit service for persons with disabilities. Needs assessment for specialized transportation is screened by the Milwaukee County Department on Aging. If eligible a Transportation I.D. Card will be issued to you.
To apply for a transportation I.D. Card or to obtain additional information about this program or any other services for older adults, please call the Aging Resource Center of Milwaukee County at (414)-289-6874 from 7 a.m. to 5:30 p.m., Monday through Friday.
A co-payment of $3.00 is required each way for rides to medical appointments. Contributions are accepted for other rides. Tipping is unnecessary.
If you have a Medical Assistance (Title 19) card and your physician approves the ride, Medical Assistance (Title 19) may cover the cost of your medical trips.
If you are enrolled in a Medicaid HMO call your provider to arrange for medical trips.
To schedule a ride for one of these purposes, call TRANSIT EXPRESS, Inc. at (414) 264-7433. A brief recorded message will be heard. Press "4" during the message to reach the operator who will assist you in scheduling your ride. If you have a rotary phone, stay on the line; someone will assist you after the recorded message ends.
