Parkinson’s and ALS. Home Hospice services are also provided to people living in the western half of Milwaukee County. The service area boundaries are 60th street west to 124th street, and from County Line Road south to College Avenue.
VITAS hospice team members visit patients wherever they call home (private home, nursing home, assisted living community, long-term care facility, etc.) in Southeastern Wisconsin.
We have offices in Wauwatosa and Racine, and an inpatient unit in West Allis.
VITAS hospice care is wherever patients and families need it. The map below indicates our general service area.
The goal at VITAS is individual care determined by the patient’s wishes and preferences. Integrated services can be brought to the bedside by a music therapist or a Paw Pals® pet visit, as needed. With recognition from the We Honor Veterans program, Milwaukee military veterans get the support they deserve near the end of life.
