Description:
Harbor Senior Concepts has two licensed class CAN, male/female 8 bed and 19 bed CBRFs. (2800 6th Ave & 2810 6th Ave). Fees are based on a base plus level of care. Area served by these two CBRFs is Green County, however they also have 13 other sites including one at: 5330 Century Ave, Middleton WI 53562. 608) 828-9600 or (608) 828-9601.
Eligibility: Diagnosis of Alzheimer's disease or other irreversible dementia/advanced aged for the 8 bed CBRF. After the referral, the Director and RN will complete an assessment for an appropriate admission.

*Regional program Director 1X/week or as needed and a regional administrative director

We have 2 buildings, one with 19 residents and one with 8. We can give total care, give insulin, Vit 12 shots, Nebulizer treatments. We can take residents to the doctor if family can't. Hold activities 3 times a day. Residents have 2 showers per week. Clinic comes every 3 months to check everyones glasses. We go to Swiss Alps once a month to eat, residents that are able. Private room and can make their room special.
