Description:
CCFC Boscobel offers health assessments, primary care, some diagnostic labs and x-rays, prescription medications, health education, referrals to community resources, and follow-up as needed.
When needed, CCFC refers patients to specialty services that go beyond primary care. Mental health assessments and counseling, orthopedic consults, physical therapy, nutrition and diabetes management are all available locally without cost to the patient. Other special needs (oncology, rheumatology, neurology, etc.) are referred to Benevolent Specialists Physicians (BSP) in Middleton, Wisconsin.
CCFC does not provide emergency care such as for heart attacks or broken bones, does not provide obstetric services (birth and reproductive health, etc.) and does not perform surgery.
People who are uninsured and having difficulty accessing primary care elsewhere are eligible to use our clinic. Persons on Medicare, Medicaid, Badgercare, Veterans’ Insurance, or any insurance of any kind, are not eligible to be seen at our clinic and are not covered by our state liability policy.
