831 Adult family home is a new remodeled ranch style home that services 3 AMBULATORY residents. It has 1 private room and 1 shared room with amenities. 831 has contract funding with ILIFE and looking to get other funding from other MCO's in the near future.
Eligibility Requirements: Each resident that wishes to participate in the program at 831 AFH will have to become a member of ILIFE before we can start with processing paperwork.
